Much of the health care team's attention to the COVID-19 pandemic focused on the initial response of stabilizing and managing the critical illness associated with this devastating virus. Now that the initial triage and medical management has been established, patient survival rates have improved. However, patients, families and providers are left to wrestle with the long-term physical, mental, and emotional consequences of both an ICU stay, and sequelae associated with COVID-19. Research has demonstrated that COVID survivorship (i.e., Long COVID) share many commonalities with Post Intensive Care Syndrome (PICS) and Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). The presenters will use this paradigm to illustrate rehabilitation approaches that address “Long Covid” across the continuum of care. Additionally, this presentation will highlight the development of an interprofessional clinic to meet the initial surge of patients and continue to meet the needs of survivors following hospital discharge. This post-ICU clinic created a telehealth model for initial intake visits with referral to necessary services depending on patient needs.
